What Are Dermal Fillers?

Most of the signs of aging we experience show up in the face—cheek depressions, frown lines, wrinkles, and crow’s feet—and are the result of a loss of facial volume. With every year you age, your body produces less collagen and elastin, which are two important structural proteins found in layers of your skin and in connective tissues.

Dermal fillers are approved by the U.S. Food and Drug Administration (FDA) as cosmetic products to help sculpt and shape areas that have lost significant facial volume. The result of dermal filler treatments is a more natural, smooth, youthful look.

 

Dermal fillers are popular because they are both safe and convenient. They’re made from three basic components that are all naturally occurring substances: biosynthetic polymer, collagen, and hyaluronic acid (HA). In the U.S., dermal fillers are only allowed to be applied by licensed physicians, physician assistants, and nurses (usually with additional certifications and training). Although dermal fillers do not produce permanent results, some dermal fillers produce results that will last for more than a year.

  • Sculptra®: This is an FDA-approved injectable made up of poly-L-lactic acid (PLLA), a substance designed to promote collagen growth over time. Results with Sculptra® typically last at least 2 years.
  • Radiesse®: Radiesse® is made from calcium hydroxyapatite (CaHa), which is a substance similar to the calcium in bones and teeth. Radiesse® can also be used in the hands to replenish lost volume, and typically produces results that last 9-15 months.

In addition to a wide range of dermal fillers, we use blunt tip cannulas for facial filler injectables to enhance your comfort and results. Blunt tip cannulas produce little or no bruising, they are known for causing less pain, and they allow us to deliver fillers to several areas with a single injection site.

When performed by a well-trained, experienced professional, such as a Board Certified Facial Plastic Surgeon or certified nurse injector, dermal fillers carry minimal risks. It is common, however, to experience swelling and bruising at the injection site. Lumps and bumps may also occur, but these typically resolve over time. Other potential risks include rare problems like occlusion of a blood vessel with tissue loss or even extremely rarely, loss of vision or stroke.

Additionally, there is the well-known, but uncommon, “delayed or late” filler reaction that occurs in association with viral illnesses in some patients with pre-existing dermal fillers. While the incidence of this reaction is not reliably established, it is not common, and is readily treated with oral steroids.

Injectable dermal fillers use varying formulations and active ingredients, making them unique and capable of addressing numerous concerns. Therefore, there is no “best” all-around filler. The filler that is right for you and your aesthetic goals will be the one that is best for you. BOTOX® is a purified form of the botulinum toxin that temporarily relaxes wrinkles or line-causing muscles—specifically, the wrinkles caused by facial movements. The active ingredients in dermal fillers are used to physically fill in hollows, wrinkles, and areas where volume has significantly diminished. Filler results can be visible immediately, while BOTOX® may take 5 to 7 days to be realized.

Sculptra® is made of poly-L-lactic acid and is more liquid in its consistency. Once injected, it incorporates into the surrounding tissues and stimulates your body to produce its own collagen. Three to four Sculptra® injections, one month apart, may be necessary to achieve the desired look. Although results are not immediate, they may last as long as 2 years. Most other fillers currently on the market are made of hyaluronic acid, a substance that occurs naturally in our bodies. It is more gel-like in its consistency and is designed to hold its shape in the area injected. The advantage of these types of fillers over Sculptra® is that the results are immediately visible. However, they are not as long-lasting.

Yes, Sculptra® can be used in conjunction with other fillers to achieve a more youthful appearance. They should not, however, be injected in the same areas of the face.

Sculptra® has to be reconstituted and injected in a very specific way to avoid nodule formation. Nodules are small lumps that can develop under the skin. They are frequently not visible but can be felt with the touch. At Faces, we reconstitute Sculptra® ahead of the visit and warm it to avoid problems with nodule formation, but with these precautions, it is rare to have this problem. The main problem with Sculptra® is that the results take time to become apparent and patience is needed.

The fillers we use at Faces are biocompatible, meaning they are not harmful to your body in any way. Many popular fillers, such as RHA, Juvéderm®, and Restylane®, are made of synthetic hyaluronic acid. This substance occurs naturally in your body and is responsible for keeping your skin plump and firm. Over time, the ingredients in injectable fillers will break down and be naturally eliminated from your body through its normal metabolic processes.

Filler injections in certain areas such as the lips can be painful, so we almost always use a topical anesthetic to make you more comfortable. Our fillers come pre-mixed with lidocaine, a mild anesthetic, so most patients find their treatment very tolerable. We also encourage cold compresses to help reduce any discomfort and swelling after your dermal filler treatment.

While they are generally considered to be affordable, there is no blanket price for injectable treatments. Each patient will have a unique treatment plan, and the costs will reflect each component. The varying factors include the size of the treatment area, the number of injections, the amount of product used, and more.
